When you visit us, you’ll be warmly greeted by our front-office personnel. If this is your first time visiting us, please bring your completed new patient forms to your appointment so we can get to know you better.

After a thorough exam and consultation, we will explain our findings and suggest treatments. We’ll describe your options and work with you to determine the best path forward. Together, you’ll create a treatment plan personalized for your unique situation.

Payment and Insurance Information

We accept cash, checks, and all major credit cards. Third-party financing, often with no or low interest for qualified applicants, is available through CareCredit. Please see the website for details.

We work with all PPO plans, and we’re in network with Delta, Cigna, and United Health. Our front-office staff will electronically file your insurance claim and work with your insurance company to maximize benefits.
